Provider enrollment attempts to ensure that only qualified and eligible individuals and entities are enrolled to receive reimbursement for services furnished to beneficiaries. NHIC, Corp. is the Part B Medicare carrier for California so all providers and suppliers who wish to receive Part B reimbursement in  this state must first enroll with NHIC, Corp. We will process your application, request additional information if necessary, determine all submitted individuals and entities are qualified and eligible to receive benefits, and assign your Personal Identification Number (PIN).

This section of our Web site provides information to help make your enrollment process as easy as possible. It includes links to articles, information on opt-out and participating providers, forms and information on choosing the correct forms, frequently asked questions, information for special category providers and suppliers and contact information.

For assistance in completing the Provider Enrollment Forms, please refer to:

In addition to the helpful information provided in this section we also encourage to visit the Centers for Medicare and Medicaid Services’ (CMS) Centers for Medicare and Medicaid Services’ (CMS) Enrollment Web site for additional information on provider enrollment.
