To look up information by CPT code, use the CMS search page for all LCDs, NCDs and Articles.  Brief instructions on using the CMS search page.

Please Note:
CMS requires that NHIC, Corp. post separate links for Northern and Southern California. However, the Local Coverage Determinations (active, draft, and future) and Articles for both sites are identical.
