  • CMS CERT Web Site - Information on the CERT program presented by the Centers for Medicare & Medicaid Services (CMS). This site provides an overview of the program and links for the following: Error Rate Reports, Over-Utilized Codes List, CERT Program Information, CERT Contact Information, Program Safeguard Contractor (PSC) Information, and Medicare Integrity Program (MIP) Information.

  • CERT Provider Web Site - The CERT Documentation Contractor (CDC) Web site is available for providers to review address information the CDC currently has in the CMS database. This is a "read only" site with an email link (byron@certcdc.com) to send updates to the CDC. Providers may also call a Customer Service Representative (CSR) at 301-957-2380 to update information. Verification of the information will be done via a return telephone call to the provider's office to ensure authenticity.

    Please review the information. In particular, the CDC needs the address where the medical records are stored and the point of contact name, direct telephone number, and fax number. If updates or corrections are required, providers may click on the email link or call a CSR to update their contact information. Items to include: affiliated contractor name and number, provider number, name of provider, address of provider (where medical records are stored), point of contact (POC) for obtaining medical records, direct telephone number of POC, and fax number of POC.
